Hip replacement surgery is one of the most successful treatments for advanced hip arthritis, offering excellent pain relief and improved mobility. For most patients, a standard “off-the-shelf” implant is highly effective. However, not everyone has typical hip anatomy.

Patients who are exceptionally tall, short, slim, or who have had previous hip trauma or congenital abnormalities may not achieve the best results with a standard implant. In such cases, a custom-made solution can offer a better fit, more natural joint mechanics, and improved long-term outcomes.

What is a custom hip replacement?

A custom hip replacement uses CT scans and 3D computer modelling to create an implant tailored to your bone. Standard implants come in set sizes, while custom implants mirror your exact anatomy.

Who is it suitable for?

You may benefit from a personalised hip implant if you have:

  • Abnormal hip anatomy or past trauma
  • Congenital hip conditions (e.g. hip dysplasia)
  • A very small or large build
  • Are aged under 60 or have high activity levels
  • Some patients simply prefer a more tailored option — and that’s absolutely valid.

What is the process for getting a custom hip replacement?

Once you’ve had your initial consultation, we arrange a specialised CT scan of your pelvis and femur. This scan is used to create a detailed 3D model of your hip, which is sent to the implant design team. They use this to manufacture a bespoke femoral component that matches your unique anatomy. The design and production process typically takes 4 to 6 weeks, and your surgery is scheduled to align with the availability of the custom implant.

Is the entire hip joint custom-made?

Not entirely. Only the femoral component (the thigh bone side) is custom-made. The acetabular component (the hip socket) is a standard, high-quality implant that your surgeon selects based on what they routinely use and what’s most appropriate for your case. The choice of bearing surfaces (the materials that glide against each other in the joint) also depends on patient-specific and surgical factors. My preferred combination — and one supported by excellent long-term data from the National Joint Registry (NJR) — is a ceramic head paired with a polyethylene liner, which offers excellent wear resistance and proven durability

Does surgery take longer than a standard hip replacement?

No. In fact, surgery time is often slightly shorter. Because the surgical plan is pre-determined and uses a single custom rasp, the procedure is efficient and precise.

Is recovery different from a standard hip replacement?

Recovery time is generally similar. However, a better-fitting implant may lead to faster early recovery and improved long-term performance, especially in active individuals.
